Care4ME community initiatives reflect our commitment to strengthening care support through thoughtful, community-driven efforts. These initiatives represent areas where we are actively developing, piloting, or exploring programs to better support caregivers, families, and individuals navigating complex care needs.

All community initiatives evolve based on identified needs, partnerships, and available resources.

Current Initiatives

ALTCS Prescreening Support — Arizona Pilot

Care4ME is currently piloting an ALTCS Prescreening Support initiative in Arizona. This initiative focuses on education, preparation, and care navigation for individuals and caregivers who are considering the Arizona Long Term Care System (ALTCS).

The goal of this initiative is to:

  • Help families better understand ALTCS before applying
  • Reduce confusion during care transitions
  • Support informed decision-making
  • Improve care navigation experiences

This initiative is offered on a limited pilot basis and may evolve as partnerships and capacity develop.

Developing Initiatives

Caregiver Education & Workshops

Care4ME is developing caregiver education initiatives designed to provide practical guidance, emotional support, and skill-building opportunities for caregivers.

Planned focus areas may include:

  • Navigating care transitions
  • Understanding long-term care options
  • Communicating with care teams
  • Caregiver self-care and sustainability

These offerings may take the form of workshops, informational sessions, or small group discussions as resources allow.

Expanded Care Navigation Support

Building on our core programs, Care4ME is exploring ways to expand care navigation services to support families earlier and more effectively in their care journeys.

This initiative may include:

  • Additional navigation tools and checklists
  • Enhanced referral pathways
  • Improved coordination with community partners
