ALTCS Prescreening Support (Arizona Pilot)
The ALTCS Prescreening Support pilot helps families and caregivers in Arizona better understand the Arizona Long Term Care System (ALTCS) before pursuing a formal application. Care4ME provides education, explanation of the process, and care navigation support to help you feel prepared and informed during care planning and transition periods.
This service is informational and supportive — it does not determine eligibility or replace official state processes.
Healing & Emergency Support
Care4ME Healing Fund
The Care4ME Healing Fund offers limited financial assistance and care coordination support to individuals and families facing extraordinary medical or care-related challenges. Assistance is provided based on need and resource availability, and may include guidance, referrals, and grants where appropriate.
This program is designed to help people access care resources and reduce barriers during difficult care journeys.
Caregiver Support & Education
Caregiver Support & Resources
